Abstract

A comprehensive search of the congestive heart failure (CHF) disease management literature was conducted to identify reports concerning the impact of educational interventions on heart failure treatment and management. They described multiple disease management interventions, which varied in style across healthcare delivery sites and providers. An analysis of the penetration, trends, and growth of CHF disease management programs, with examples of current programs used by MCOs and state Medicaid programs, is provided. This article also discusses why heart failure is often selected for disease management intervention, the management and treatment of heart failure, and a review of heart failure disease management literature.
