Abstract

We describe a case of infective endocarditis (IE) caused by Tropheryma whipplei, the causative agent of Whipple’s disease. A 73-year-old cattle farmer presented with 12 months of exertional dyspnoea and a heart murmur. His past medical history and initial investigations were unremarkable. A transthoracic echocardiogram (TTE) revealed severe aortic regurgitation and moderate stenosis. Blood cultures taken prior to antibiotics were negative as were serologies for causes of culture negative IE. Cardiothoracic surgery revealed pericardial inflammation and a large vegetation on the left coronary cusp that was debrided.
