Abstract
A 62-year-old man was admitted to our hospital because of severe pain and purple discoloration in the first toe of left foot for 2 weeks. He was diagnosed with type 2 diabetes 10 years ago, but his blood glucose was poorly controlled with metformin. He had no history of hypertension or smoking. Physical examination showed that his left great toe was painful and purple with low skin temperature. The dorsalis pedis artery pulse was impalpable.
