Abstract
Objective To explore the effect of WeChat-based care continuous care on discharged patients with liver failure. Methods A total of 124 patients with liver failure who were treated in our hospital and discharged after recovery from January 2014 to June 2015 were selected as the research objects and randomly divided into the intervention group and the control group.All the patients of the two groups received regular health education and self-management guidance during hospitalization, and the baseline assessment was completed before discharge.After discharge, the patients of the control group received the regular follow-up of the hospital, while the patients of the intervention group were included in the WeChat-based management group which periodically sended disease-related health knowledge and informations and provided personalized interventions through interaction as well.The evaluation was conducted again 6 months after discharge. Results There was no statistical significance in terms of the differences of the general information between the two groups (P>0.05). The mastery of the disease-related knowledge, medication adherence and subjective well-being of the patients of the intervention group were better than that of the control group, and the difference was statistical significant (P<0.05). Conclusion WeChat-based continuous care for discharged patients with liver failure proved better effect and is worthy of promotion. Key words: WeChat; Liver failure; Patients; Continuous care
