Spontaneous thrombosis of varicocele is an extremely rare cause of acute scrotal swelling. Herein, we report a case of same in a young adult serving soldier ofIndian army who presented to the emergency department with a history of sudden onset left scrotal swelling of 2 days duration. There was no history of trauma. All his haematological and biochemical parameters were normal except elevated hepatic enzymes alanine aminotransferase (ALT) and aspartate aminotransferase (AST). The diagnosis was clinched on colourdoppler flow imaging (CDFI) and ultrasonography (USG) of the testis and scrotum. He was managed conservatively with complete clinical and radiological resolution within 2 weeks.
