Abstract
The quality of healthcare needs to be improved, both in terms of BPS information system, recording system, support system and health service support system. In the Health Service administration system can be carried out process of logging for patients who visit by doing a recording that includes the activity of patients. Documents containing a visiting patient's history are called BPS. Analysis of data needs related to BPS. The software is built using the programming language MySql, Vb.Net and Cristalreport. The purpose of making this application is to know the system that runs in the process of logging the patient in the BPS "Fitri Alatif" before the design of a computerized BPS information system "Fitri Alatif", so that it can be built a Recording information systems and reports using the desktop-based prototype method of a private practice midwife and using Unified Modeling Lenguage (UML) for system modeling. Where this system can be used to facilitate the management of BPS in the recording and report of patients who visit on a considerable scale.
