Abstract

A patient with unnoticed lithium-induced diabetes insipidus underwent ulcer surgery and developed postoperative water depletion and hyperosmolar coma. Vigorous fluid replacement was instituted, but the urinary volume increased to over 38 liters/24 hours. Vasopressin therapy was ineffective, but hydrochlorothiazide therapy resulted in a rapid reduction in urinary volume.
