Abstract
We combine with the present situation about the serious imbalance of medical resource utilization in Chinese referral system and study the service capacity design and the optimal referral rate determination of the community hospital under the hierarchical medical system. We establish an integrated model of the queuing and game theory, and analyze the equilibrium behavior of delay-sensitive patients. Class 3-A hospital fixed the service capacity and its service level is high enough. According to the total number of potential patients and class 3-A hospital's service capacity, government design the community hospital's service ability and determine the optimal referral rate from the perspective of the total social cost minimization. Under the condition of the patients' equilibrium behavior, the community hospital's optimal service capacity is a convex about the total potential patients and increases with the increasing of class 3-A hospital's service capacity; the total social cost is a convex about the service capacity of class 3-A hospital and increase with the increasing of the total number of potential patients. Thus, it is important to determine appropriate service capacity and service level of hospitals, and determine the appropriate treatment area of hospitals.
