Abstract

A 37-year-old man with an extensive psychiatric history attempted self-castration by tightening a coat hanger around his scrotum. This action resulted in scrotal cellulitis. Scrotal imaging with Tc-99m pertechnetate was requested to assess testicular viability. The study shows increased activity surrounding the region of the coat hanger and extending inferiorly; activity was greater on the right side. An ovoid area of relatively decreased activity on the left was interpreted initially as left testicular infarction. Subsequent surgical exploration showed that both testicles were perfused normally. The relatively decreased activity on the left presumably represents an area of less severe cellulitus.
