Abstract

Maintenance is the process of repairing, correcting, restoring, and modifying equipment for the purpose of maintaining the operability, reliability, and safety of equipment and systems. In chemical-process facilities, maintenance of electrical and process control systems is an important safety issue. A significant percentage of safety incidents are attributed to improper or no maintenance of these systems. In one incident, a plant stopped cleaning substations, and after a time, dust accumulated in switch gear enclosures. During a severe rainstorm, the dust accumulation became moist and caused a voltage flashover from the switch gear bus bar across the insulation and to the switch gear enclosure. The flashover caused the circuit breakers to operate, which eventually shut down the entire plant.
