Abstract

JTOF is a benign fibro-osseus lesion of aggressive clinical appearance, located mainly in maxilla and mandible, described in children between 8,5 and 12 years old. Trabecular (JTOF) and Psammomatoid (JPOF) Ossifying Fibromas are classified in the group of Fibro-osseous and osteochondromatous lesions according to WHO Classification (2017). Tumour does not affect the bone structure but due to its expansive growth may lead to facial asymmetry. In the described case, a 12-year-old boy was admitted to the dental office with a chief complaint of painless hard swelling of the hard palate which appeared about three weeks before and grew rapidly. Clinical examination revealed an asymptomatic, hard, well defined, pedunculated lesion connected to the tissues of marginal periodontium. Radiological examination did not reveal any pathologies. Surgical excision was performed, as the clinical appearance suggested fibrous epulis. Further histopathological examination with immunohistochemical staining revealed features of Trabecular JOF. No recurrence was observed in the follow up of three years. Described case of JTOF indicates that the lesion might be located extra-osseus in the soft tissues of the hard palate. JTOF should be taken into consideration in the differential diagnosis of pedunculated lesions of the mucosa among fibromatous epulis or peripheral ossifying fibroma (POF). Patients diagnosed with JTOF should be monitored both clinically and radiographically due to the high recurrence of the lesion.
