Abstract
The purpose of the chapter is to present the application of fuzzy Multi-Criteria Decision-Making (MCDM) approach within failure mode effect analysis (FMEA) approach for assessing the problems associated with the failure of cheese packaging machine (CPM). FMEA approach was executed to list failure causes under CPM and fuzzy ratings for three risk criteria, i.e., probability of failure occurrence (1), severity (2) and detection (3) are collected against the listed failure causes through expert’s feedback. For ranking of reported failure causes, the Fuzzy Combinative Distance-Based Assessment (FCODAS) approach was deployed within a fuzzy FMEA tool. With the usage of the proposed MCDM technique, the failure causes winding failure of conveyor motor (PKM14) under CPM was identified as the most critical. The analysis-based ranking results were given to the plant’s maintenance manager, who expressed his consent with the obtained results.
