Abstract
For the past 6 years, the author has used two modifications of the rotation-advancement repair for unilateral cleft lips in 126 consecutive patients in an attempt to restore and preserve the normal curved lines of the lip's anatomy. The vertical limbs of the rotation and advancement flaps are curved to compensate for the distortion caused by the unopposed pull of the orbicularis oris muscle. A small triangular flap incorporating the vermilion-cutaneous roll restores continuity of the roll across the repair and helps restore and maintain the profile of the lip.
