Abstract
Dartmouth-Hitchcock Medical Center's Heart Failure program consists of one dedicated Heart failure physician, 2 Heart Failure Nurse Practitioners, 2 RN Care Managers, a Dietitian, and 2 Administrative Assistants. The population is rural, coming from Eastern Vermont, Western Maine, most of NH and largely a Medicare population. We provide inpatient consultative care and education, with a dedicated outpatient clinic for heart failure patients.
