Abstract
This is an interesting case with excellent imaging to support an anatomic and biochemical pathology.1 However, in the absence of progressive decline in cognitive function, can a diagnosis of Alzheimer disease really be made? Either another decade is needed to see whether the patient's pathology generalizes throughout the brain, or this case should be reclassified as a focal tauopathy.
