Abstract

Postoperative pain is a major factor in decreased patient satisfaction after Penile Prosthesis (PP) Surgery. Multiple protocols were developed in order to lower narcotic use, with varying success rates. Therefore, we tried a novel intraoperative anesthesia regimen in 420 cases with age range 35 to 87. The aim of this retrospective study is to assess the effectiveness of combined anesthesia in decreasing postoperative pain and subsequent need for narcotic use in men undergoing PP surgery. Retrospective analysis of data was taken from 420 patients who underwent malleable and inflatable penile prosthesis surgeries from February 2016 to October 2019. Ninety four percent of patients (397 out of 420) suffer from co-morbid conditions, mainly diabetes, hypertension and ischemic heart disease (IHD). Our method involves using spinal anesthesia with 1 ml of Bupivacaine 0.5% at L3-L4 followed by caudal block using 15 ml of Bupivacaine 0.5% at the caudal area through sacral hiatus.
