Abstract

Objective: Although fetal ovarian cysts are common, congenital ovarian cysts torsion may be hazardous. We present a case of a fetal ovarian cyst torsion detected prenatally by serial ultrasonographic assessment and postpartum management. Case Report(s): A 19-year-old female at 34 weeks’ gestation was referred to our hospital because of fetal abdominal cyst. The two-dimensional and three-dimensional ultrasound showed a fetal ovarian tumor (5.0×4.7×4.4cm) with cystic shadow. At 36 weeks’ gestation, the tumor was smaller (5.5×3.2×2.4cm). However, at 38 weeks’ gestation, the tumor could not be detected by ultrasound and ovarian cyst with torsion was suspected. After birth, the newborn received abdominal ultrasound and computerized tomography. Right oophorectomy was performed postnatally. The pathologic result revealed follicular-lutein cyst with torsion. Conclusion(s): Congenital ovarian cysts usually have a good prognosis. However, the cyst may undergo complications such as torsion and rupture. When a fetal ovarian cyst with torsion is suspected, serial ultrasound examinations should be performed prenatal, and complete investigation as well as surgical consultation should be undertaken immediate after birth to prevent further complications.
