Abstract

Objective To investigate the status of diagnosis and treatment of pulmonary embolism in hospitals of Chongqing in recent ten years in order to provide evidences for the rational organization of pulmonary embolism training and improvement of the level of pulmonary embolism diagnosis and treatment. Methods Samples from 30 hospitals in Chongqing(15 tertiary hospitals,15 second-class hospitals) by convenience sampling method were investigated. The investigation included number of patients with pulmonary embolism from 2003 to 2012,medical conditions,prevention and screening of pulmonary embolism. Results ①The total number of pulmonary embolism diagnosis cases in nearly ten years was 703(512 cases in tertiary hospitals,191 cases in second-class hospitals),of whom 581 cases were diagnosed in clinic,122 cases were confirmed.The definite diagnosis rate was 17.35%.②With the time,pulmonary embolism diagnosis cases increased gradually,dramatically increased in 2009- 2012.③The unmber of hospitals in which pulmonary embolism could be diagnosed had increased year by year except six hospitals.④All of the investigated hospitals could carry out biochemical and imaging examinations such as D-dimer, blood gas analysis, myocardial injury markers, activated partial thromboplastin time,international normalized ratio,lower limb vascular ultrasound,cardiac ultrasound, and enhancement CT.All the investigated hospitals had the essential medicines,such as heparin,low molecular heparin,warfarin,and urokinase.⑤17 hospitals could conduct CT pulmonary angiography, including eight hospitals for clinical diagnosis.⑥The eight investigated hospitals conducted pulmonary embolism risk assessment with relevant scale table in some departments. Conclusions The consciousness of pulmonary embolism diagnosis has been gradually improved for the most hospital medical staffs from 2003 to 2012 in Chongqing,but the consciousness of prevention and screening high-risk patients with pulmonary embolism is poor.In some hospitals pulmonary embolism diagnosis is mainly conducted in clinic,and the definite diagnose rate is low.So the ability of diagnosis and treatment of pulmonary embolism should be further improved.
