Abstract

The purpose of this study is to evaluate the effectiveness of rehabilitation with the use of virtual reality technologies and the influence of the main clinical and functional parameters on the functional outcome and the effectiveness of VR-therapy in patients with different degrees of cognitive decline in the acute period of ischemic stroke Materials and methods 120 patients diagnosed with ischemic stroke with cognitive decline according to the Montreal Cognitive Assessment Scale (MoCA) were examined. Patients were divided into groups: 1st - 62 patients with moderate cognitive impairment (20-25 points), 2nd group - 58 patients with dementia ( 20 points). On 4th to 5th day rehabilitation using virtual reality (VR) was performed. The following scales were used to examine cognitive and functional status: Barthel Index (BI), Rankin Scale (mRS), NIHSS; tests to assess cognitive functions; neuroimaging scales ASPECTS, STRIVE and MTA. The functional outcome was calculated by the difference between mRS scores on the 1st and 14th days - Δ mRS, rehabilitation effectiveness (RE) - VR-system parameters before and after treatment - Δ RE. Results Significant improvement was demonstrated in all cognitive and functional parameters except semantic information processing, attention and constructive praxis in the 1st group. In the 2nd group, improvement was noted in NIHSS, speech and mnestic disorders. In the 1st group, correlation of Δ RE was found with lesion size, attention level, BI, mRS and NIHSS; in the 2nd group correlation was found with age, gender, degree of IQCODE and STRIVE, presence of atrophy, semantic aphasia and perceptual impairment. In the 1st group, Δ mRS was correlated with the size and side of the lesion, NIHSS, ASPECTS, attention and speech function; in the 2nd group - with parameters of semantic information processing, constructive praxis, degree of IQCODE, size of the focus, ASPECTS and BI. Conclusion Modern approaches to VR-rehabilitation of patients with post-stroke cognitive impairment require the development of individual methods using immersion environment focused on the structure, etiology and severity of cognitive deficit taking into account the degree of the patient's functional state.
