Abstract
Among patients with persistent atrial fibrillation (AF), there is limited data to support posterior wall isolation (PWI) in addition to pulmonary vein isolation (PVI) to decrease rates of recurrence in persistent atrial fibrillation. There is emerging data to suggest the recurrence rate of atrial fibrillation with cryoballoon ablation is lower in comparison to radiofrequency ablation in patients who underwent contaminant PVI and PWI. To evaluate recurrence rates of AF in patients with persistent AF with concomitant PVI and PWI with cryoballoon ablation in comparison to radiofrequency ablation at single a center. From December 2020 to August 2022, 25 and 98 patients with persistent AF who underwent contaminant PVI and PWI with cryoballoon and radiofrequency ablation, respectively, were included. Patient demographics and recurrence rates were retrospectively analyzed (Table). After mean follow-up of 13 months, recurrence of atrial arrhythmia was lower with cryoballoon ablation than with radiofrequency ablation, which was not statistically significant (2 of 25 [8%] vs. 14 of 98 [14%], p-value 0.52). Both patients in cryoballoon ablation group had recurrent atrial fibrillation. In the radiofrequency group, 11 of 14 (79%) patients had recurrent atrial fibrillation, and 3 of 14 (21%) reoccurred with an atypical atrial flutter. Three patients in the radiofrequency group underwent repeat ablation procedure after recurrence. Repeat electroanatomic mapping demonstrated: focal reconnection in PVI (patient 1), PWI (patient 2), and a mitral annular flutter (patient 3). Among patients with persistent atrial fibrillation, there was a trend towards a lower rate of atrial arrhythmia recurrence with cryoballoon ablation in comparison to radiofrequency ablation in patients who underwent concomitant PVI and PWI.
