Abstract
The plasma half-life of unchanged paracetamol was determined in thirty patients with paracetamol overdosage. Hepatic necrosis was observed in seventeen patients, one of whom developed fatal hepatic coma. The metabolism of paracetamol was impaired in patients with hepatic injury and there were statistically significant correlations between biochemical indices of liver damage and paracetamol half-lives. The plasma-paracetamol half-life was the most reliable early guide to prognosis, and liver damage is to be expected if the half-life exceeds 4 hours.
