Introduction

The primary barriers in ensuring and achieving a sustainable patient safety culture among hospitals are adverse events such as medical malpractice and medication errors. In Saudi Arabia alone, adverse events related to clinical malpractice remain the most occurring medical error related to patient safety (Alharbi et al, 2019). The incidence of medical errors and litigations involving medical malpractice have been increasing in this country. In many parts of the world, the need to determine the state of patient safety has become an urgent need. The Hospital Survey on Patient Safety Culture (HSOPSC) was released in January 2016 by the Agency for Healthcare Research and Quality (AHRQ). Several studies were undertaken to determine the attitude, perception, and awareness of patient safety culture across the world using tools such as the AHRQ survey
