Abstract

In this chapter, we discuss pancreatic cancer, the fourth most common cause of cancer-related death in the United States. As in the prior chapter, the discussion is centered on pancreatic adenocarcinoma given that it is synonymous with pancreatic cancer. This chapter reviews the clinical presentation and management of the disease before introducing the reader to the embryological development, the normal anatomy, and the normal histology of the pancreas. The histopathologic features of invasive disease and its precursors are subsequently described. The remainder of the chapter focuses on the molecular alterations in pancreatic cancer, discussing the most common somatic alterations that lead to carcinogenesis, and reviewing the majority of germline alterations implicated in the disease. An understanding of the genetic and molecular alterations underlying pancreatic cancer is crucial in guiding future molecular studies and in determining the subtypes that may respond to novel therapeutics.
