Abstract
Gastric pouch creation during laparoscopic Roux-en-Y Gastric Bypass (LRYGB) requires access to the lesser sac. This is usually achieved by developing a perigastric plane between the lesser curve of the stomach and its mesentery. Frequently, this is tedious and leads to complications such as gastric wall injury and bleeding. Approaching the lesser sac through the transparent portion of the gastrohepatic ligament (pars flaccida) allows for an easier and safer approach. This technique, however, results in transection of one of the gastric branches of the anterior nerve of Laterjet.
