Abstract

ABSTRACT Introduction Colon cancer accounts for number of cases, a superior of the cause of death like gastric cancer, lung cancer. Here we report aggregates the multiple neoplasms including hematological malignancy and colon cancer. Methods In the case that hematologic malignancy was diagnosed in our hospital by 1988 from 2000, we intended for 211 multiple primary cancer that was complication other malignancy. The multiple primary cancer including colon cancer and the hematologic malignancy was 32 cases. So we reviewed and reported about age, gender, kind of hematologic malignancy, type, prognosis and the cause of death. Results The subjects were 32 cases including female 12 cases, male 20 cases, synchronous type 13cases, metachronous type 19 cases. Multiple neoplasms consist of double cancer 23 cases, triple cancer 8 cases, quadruple cancer 1 case. The median age of first diagnosis and second diagnosis were 65.5ys (ranged 44-81ys), 62ys (ranged 44-86) respectively. Hematological malignancy consist of malignant lymphoma 18cases, myelodysplastic syndrome 4cases, chronic myelogenous leukemia 3 cases, multiple myeloma 2cases, macroglobulinemia 2 cases, acute lymphoblastic leukemia 2 cases, acute myelogenous leukemia 1case. The sites of colon cancer studied 29 cases and results were that ascending colon 7cases, transverse colon 3 cases, descending colon 2 cases, sigmoid colon 9cases, rectum 8cases. There were 20 cases that was revealed pathological findings. Poor differentiated adenocarcinoma was 1 case, moderate differentiated adenocarcinoma 14cases, well differentiated adenocarcinoma 5cases. The interval time of metachronous type, the median interval time was 58M (ranged 12-181M). In 19metachronous type, double cancer was 13 cases and triple cancer was 5cases. In a double cancer, six cases were preceding colon cancer and 7 cases were preceding hematological malignancy, an interval of the colon cancer precedence cases, the median time was 101M(ranged 38-181M), hematological malignancy precedence cases was 27M(ranged12-96M). Conclusion Colon cancer is important component of the multiple primary cancer that including hematological malignancy such as gastric cancer, lung cancer. In the case of hematological malignancy precedence case, 4 of 7 colon cancer occurred within three years. At the staging period or final clinical outcome examination, better to examine about colon by endoscopic fiber. On the other hand, colon cancer precedence case, median time was 101M from diagnosis of hematological malignancy. Predict the occurrence of colon cancer from hematologic malignancy, it is not useful. We should take attention about incidence of other malignancies, particularly those that occur from gastrointestinal tract and serious observation about distant metastasis site, such as liver and lung.
