Abstract

Purpose: Ensuring quality transitions of care between the hospital and outpatient settings is of paramount importance. At Massachusetts General Hospital for Children (MGHfC), pediatric hospital medicine (PHM) physicians communicate with patients’ primary care pediatricians (PCPs) at discharge without additional patient communication post-discharge. Telemedicine in the form of virtual visits (VV) provides a platform for direct patient-provider communication that has not been previously investigated within this specific setting.
