Abstract

Background and Objectives: Ensuring quality transitions of care is challenging. Multiple post-discharge issues including missing appointments and prescriptions have been identified in large numbers of hospitalized (Rehm et al. J Hosp Med. 2018). Benefits of post-discharge telemedicine, or virtual visits (VV), on these issues have not been previously studied in a general pediatric population.
