Abstract
Chronic diseases cause 70% of deaths worldwide, including 15 million premature deaths per year. The chronic care model was established to improve chronic disease management; however, it lacks a systematic approach to assist clinicians in fully assessing chronic care needs. OLD CLASS is a method developed by nurse practitioner faculty to guide nurse practitioner students to collect a thorough history, including the lived experience of chronic illness, so they can provide high-quality chronic disease management.
