Abstract

Spontaneous pneumomediastinum is an underdiagnosed pathology, a process with no apparent demonstrable cause and almost always characterized by chest pain, dyspnea and subcutaneous emphysema on physical examination, it is diagnosed by chest radiography or thoracic computed tomography, with a favorable evolution in the vast majority of patients. Three cases of spontaneous pneumomediastinum diagnosed at the Vithas 9 de Octubre hospital in Valencia, Spain, are presented. All were young patients who came to the emergency department seeking a second medical opinion. These cases had an atypical presentation, the main symptoms were cervicalgia, dysphonia and odynophagia caused by retropharyngeal emphysema secondary to pneumomediastinum. Lateral cervical radiography was essential to make the diagnosis, which was corroborated with chest computed tomography. In all cases the evolution was good after a few days in the thoracic surgery department of the hospital.
