Abstract

Abstract Nasocardiac reflex is a trigeminovagal reflex that occurs during procedures in and around the nasal mucosa. It is characterized by a chain of events similar to vasovagal syncope. A case of nasocardiac reflex developed intraoperatively during augmentation rhinoplasty in a young female being operated on for saddle nose deformity. The occurrence of transient vagal response during surgical dissection of the nasal septum was evident in the form of severe bradycardia and hemodynamic instability. To complete the surgery without undue consequences, the situation was managed with transient stoppage of surgery, followed by intravenous atropine and topical infiltration of the involved nasal mucosa with 10 ml of 2% lignocaine.
