Abstract
乳癌術後転移性肺腫瘍に対し外科的切除，薬物療法のいずれを選択するかについては議論が多い．当院で外科的切除が行われた乳癌術後転移性肺腫瘍9症例についてその治療成績を検討した．孤立性結節が7例あり，術前診断として4例が原発性肺癌の可能性を疑われていた．乳癌切除から肺転移切除までのDFI（disease-free interval）は平均5.4年（1.1～12.6年），肺転移手術後の平均生存期間は61.6カ月，5年生存率は72.9％でありDFIの長い症例に長期生存例がみられた．薬物療法前後でレセプター発現に変化がみられた症例では適切な治療薬への変更が有効な症例も確認されており，再発，薬物耐性が疑われる際にはレセプターの再評価を目的とした外科的切除が有用と考えられる．
