I have recently seen a patient with a murmur thought to represent aortic insufficiency that was heard best, and sometimes exclusively, in the supine position. Serial observations documented a consistent relation between the patient's position, his blood pressure, and the audibility of the murmur.A previously healthy 12-year-old boy had the sudden onset of vomiting, diarrhea, and marked fatigue in July 1972.
