Abstract
The aim of the article is to assess the process of deinstitutionalization from the point of view of providing forms of assistance in the face of ongoing demographic changes. The first part of the article analyzes the conditions of the social assistance system in Poland. As a result of collective analyzes of the reports of the Public Opinion Research Center and the Central Statistical Office presenting aggregated research results, the structure of service providers and beneficiaries of the social assistance system was defined in terms of the scope of assistance. In the second part, an attempt was made to identify the challenges in relation to the management of mechanisms of multi-level and multi-sector coordination of deinstitutionalized care for people in post-working age, provided in the local environment from the point of view of the concept of sustainable development.
