Abstract

A case of primary early carcinoma of the upper area of the duodenal papilla was reported. A 77-year-old man was admitted to our hospital for further investigation, because an abnormality of the stomach was suspected by mass screening X-ray examination. Endoscopic examination of the stomach was no abnormal findings, but of the upper area of the duodenal papilla revealed IIc like lesion measuring 0.9x0.7cm. The endoscopic biopsy specimen showed moderately differentiated adenocarcinoma. Under a preoperative diagnosis of the cancer was limited within mucosa, only partial duodenectomy was performed. Histological diagnosis was moderately differentiated adenocarcinoma within mucosa, type IIb (INF m, α, ly0, v0, ow (-) , aw (-) ) . Under postoperative course, the patient had a symptom free life of 2 years. This case showed necessity of the screening of endoscopic examination within the second portion of the duodenum.
