Abstract

Primary early carcinoma of the duodenum excluding duodenal papilla is uncommon. We report a case of primary early carcinoma of the duodenal bulb with a review of literature.A 77-year-old man was admitted to our hospital because of upper abdominal discomfort and lumbago. Endoscopic examination of the upper gastrointestinal tract revealed an irregular and elevated lesion with nodular surface in the anterior wall of the duodenal bulb. The biopsy specimens showed papillary adenocarcinoma.Gastrectomy, partial duodenectomy and lymph node dissection were performed. Gross appearance of the resected specimen showed a nodular polypoid lesion with broad base, measuring 1.6×1.1×0.3 cm, in the anterior wall of the duodenal bulb, being about 1.5 cm distal to the pyloric ring. Histological diagnosis was papillary adenocarcinoma located within mucosa and submucosa without lymph node metastasis.
