Abstract

Team Strategies and Tools to Enhance Performance and Patient Safety (TeamSTEPPS) is one program designed to improve communication and performance across all roles and disciplines of the health care team. This paper describes the lessons learned during a quality improvement project at a Federally Qualified Health Center (FQHC) based in a Midwestern urban community. One objective was to expand collaboration and team function among the health care providers and support staff by training RN Chronic Care Coordinators to develop, facilitate and lead interprofessional teams to improve chronic care coordination. The project used RN complex care managers to help improve team communication using the TeamSTEPPS approach.
