Abstract

We present a case of a 67-year-old patient who significantly benefit from conduction system pacing. The presence of septal scar initially limited chance of success. Ineffective attempt to upgrade the ICD to CRT along with the end stage of heart failure despite optimal medical therapy, however, forced us to make an attempt to upgrade existing ICD to CSP. The procedure itself was demanding, but after numerous attempts we achieved a satisfactory electrocardiographic effect. What surprised us the most was that the clinical effect turned out to be spectacular.
