Abstract

This chapter discusses incidence, predisposing factors, and clinical course of late relapses (LR) in patients with advanced malignant germ cell tumors (MGCT), defined as recurrences occurring 24 months or more after the last course of chemotherapy. From 1976 to 1990,125 male patients with advanced MGCT received cisplatin-based chemotherapy with or without adjunctive surgery. Eight cases of LR have been recorded in series with a median follow-up of 81 months. Univariate analysis of prognostic factors in group showed that the persistence of a residual mass and the presence of teratomatous elements in the primary tumor were predisposing factors for LR. The consequences of the policy of not performing routine lymphadenectomy in early stages of nonseminomatous tumors should be investigated in the long term.
