Highlights
There was no difference in the Bristol stool types between patients with and without diverticulosis. Among those who underwent mucosal sampling, there was no increase in inflammatory cytokines such as TNFa and a reduced mucosal mRNA expression of IL-6 and IL-10 in those with diverticulosis, and no increase in CD4þ or CD57þ T cells. These findings suggest that uncomplicated diverticular disease does not cause chronic gastrointestinal symptoms and that evaluation of patients with such symptoms should seek alternate causes or focus on symptomatic treatment rather than attributing the symptoms to the diverticula
This study illustrates that patients who have downstaging of their HCC by locoregional therapy (LRT) may represent a cohort with favorable tumor biology and can be expected to have good outcomes after liver transplantation (LT)
Less is known about whether persistent inflammation in perianal Crohn’s disease (CD) is associated with an increase in risk of anorectal cancer
Summary
There is growing interest in the use of locoregional therapy (LRT) to downstage tumor to meet Milan criteria, evidence for this far has been mostly from single center studies. This multi-center study by Mehta et al[3] examined the outcomes of 187 consecutive adult patients with HCC who underwent downstaging with LRT from 3 liver transplant centers in California (50% with transarterial chemoembolization alone and 43% with transarterial chemoembolization in combination with radiofrequency ablation).
