Abstract

A conceptual model for practical investigation of occupational accidents within the industrial company is presented. The model comprises two levels of investigation. On the accident sequence level, deviations or chains of deviations are mapped. On the second, determining factors level, the production process conditions related to the accident are highlighted. A preliminary version of the model has been used in five pilot studies within different branches of Swedish industry. Results show that information existing within the company on accident sequences and determining factors, but not included in the accident reports used by the local safety organisation, could be tapped by applying the model in the investigative procedure. The authors propose to use the model in intervention projects within Swedish companies aimed at establishing new methods in local accident prevention. The analysis of accidents and the development of counter-measures will take place in a structured group setting including workers, management and staff.
