Abstract

BackgroundThis project aimed to improve adherence to postdischarge follow-up among all-cause heart failure (HF) veterans by implementing the evidence-based Focused Discharge Handoff Tool (FDHT). MethodsParticipating inpatient providers supported implementing the FDHT and enrolling veterans in the 7-day post-hospitalization telehealth and primary care appointments. ResultsTotal compliance rate with the use of FDHT was 67.3% (n = 35); remote patient monitoring home telehealth follow-up appointments improved from 18.8% (n = 8) to 50% (n = 26). The System Usability Scale survey scored below average. DiscussionUsing the FDHT to standardize the federal system’s discharge process provided insightful and promising results for supporting veterans. LimitationsDue to the small sample size, the discharge tool was not embedded into electronic medical records. Conclusion and Sustaining PlansThese data reflect positive outcomes for increasing veteran compliance with HF treatment plans by promoting 7-day telehealth appointments and safer transition of care.
