Abstract

This project involved developing an easy-to-use transition toolkit for clinics serving children and young adults with chronic illnesses who are transitioning from a pediatric model of care to an adult model of care. Got Transition’s 6 core elements served as a guiding framework for the toolkit, which was developed through an iterative process with substantial stakeholder input. The project was evaluated using a post–then survey and 3 open-ended questions. Large effect sizes (<0.8) were identified for all items evaluated. The transition toolkit was effective for implementing transition care in the participating clinics. Lessons learned through the implementation process are shared.
