Abstract

Ischemic stroke of unusual causes accounts the 8% and 30% of the strokes among young adults. Etiologic heterogeneity is its main feature; it might be the first manifestation of the underlying disease or an evolutionary complication. Underlying disease-related symptoms/sings can guide the diagnosis. Treatment relies on the management of underlying disease, if possible, and antithrombotic drugs. Cerebral sinus or venous thrombotic occlusion affects mainly young adults with predominance in women (3:1). The most common risk factors are hereditary thrombophilia, pregnancy and puerperium, oral contraceptives, neoplasms, inflammatory arteriopathies and infections. Headache is the most frequent symptom. Isolated intracranial hypertension, subacute encephalopathy and focal disturbances are the main presentation syndromes. Antithrombotic treatment is required. In the most cases, prognosis is good.
