Abstract
A 65-year-old man was admitted complaining of mild dyspnea of effort recent in onset Both ankles had been swollen for the past month or more. On physical examination, there was marked finger clubbing, bilateral pitting edema of the lower extremities, and diminished breath sounds and dullness to percussion of the right lower chest The significant laboratory findings were an elevated blood sedimentation rate of 70 mm in the first hour, a positive Weinberg complement fixation test, and a positive Casoni intradermal test Figures 1 and 2 are the admission chest films. Figure 3 is a combined technetium (99Tc) liver and lung scan. Figure 4 was made following diagnostic pneumoperitoneum. Roentgenograms of the hands and knees showed hypertrophic pulmonary osteoarthropathy.
