Abstract

Introduction: Transition from pediatric to adult-orientated care is a difficult period of life in adolescents with chronic diseases. 40% of these patients drop out from their special care with fatal consequences for their health. Structured transition programs can reduce this percentage to 10%. In Germany neither structural nor financial procuderal methods are established. The „Berliner Transitionsprogramm“ should result in a structured transition process financed by the health insurances. The „Berliner Transitionsprogramm“ is funded by the Robert-Bosch-Stiftung. Conception and realisation is carried out by DRK Kliniken Berlin and the scientific surveillance by the IGES Insitute.
