Abstract

Objective To discuss the validity and safety of treating bronchic tuberculosis combined with balloon dilating,freezing,and argon knife.Methods On the basis of painless bronchoscope,the bronchic tuberculosis was observed by superfine bronchoscope,and then the pathological change was classified based on the results of bronchoscope and the HRCT 3-D reconstruction pre-surgery.Based on the position of stenosis,proper balloon dilating catheter was selected to treat the pathological change combined with XT40 therapeutic bronchoscope.Treating was performed once to four times each week combined with dilating,freezing,or argon knife.The time of dilating was selected to be 1-4 min,and the pressure of dilating was selected to be 3-6 atmospheres,and no more than 8 atmospheres.The time of freezing was selected to be 0.5-3 min,the power of the argon knife was selected to be 25 W,and the time was no less than 5 s.Results 35 cases were totally valid post-surgery,14 cases were valid,and three cases were totally invalid.The total effective rate of the treating was 91.4%.The relevant side-effects of the three interventional therapies mentioned above were pneumothorax caused by laceration of bronchus in one case,soft freezing probe damage caused by freezing cut in one case,bronchoscope damage caused by burning in trachea during argon knife in one case,and massive haemorrhage in three cases.The incidence rate of the side-effects was 17.1% (6/35),but none was dead.Conclusions In the interventional therapy companied with bronchoscope for treating different clinical types or complex bronchic tuberculosis,it is momentous to properly choose balloon dilating,freezing,or argon knife.Although the interventional therapies mentioned above show obvious effects,it should be paid more attention to the safety.
