Abstract

Abstract Objectives This study aimed to assess drug-related problems (DRPs) and the cost implication of pharmacist intervention (PI) among hospitalized elderly patients. Methods Ward pharmacists reviewed the patients’ pharmacotherapy charts, identified the DRP, and discussed the PI with the prescriber. Descriptive analysis was performed on the data collected. Key findings More than half (58.0%) of DRPs were reported in elderly patients, which consist mostly of inappropriate drug doses or frequency (47.3%). The total estimated drug cost-saving of PI was RM 5206.76 (USD 1195.76). Conclusions PI has substantially reduced inappropriate drug therapy and contributed to saving drug-related costs.
