Abstract

Comment: The results of this study may be interpreted in several ways. It is somewhat misleading when the authors state that intubating conditions are suitable for mivacurium compared with succinylcholine. Although both showed a similar number of excellent and good conditions for intubation, the mivacurium group had a smaller percentage of excellent conditions and a larger number of good conditions. Good conditions were described as mild cough on intubation. I do not think this is acceptable, and mivacurium should not be considered comparable with succinylcholine. Mivacurium may be useful when a short procedure requires muscle relaxation and succinylcholine is not indicated. Other nondepolarizing agents such as rocuronium provide better intubating conditions compared with mivacurium in 60 sec but may have a disadvantage of lasting longer than mivacurium. The study does indicate that the hypotension (histamine release) resulting from a single dose of mivacurium may be significantly decreased with divided doses.
