Abstract

Moul 1 Moul JW. Public survey and survival data do not support recommendations to discontinue PSA screening in men at age 75. Urology. Google Scholar concludes that the United States Preventive Services Task Force (USPSTF) recommendation “to discontinue screening at age 75 was supported neither by public opinion or disease risk and survival results.” However, the findings are unlikely to represent well-informed public opinion and do not accurately portray or refute USPTF prostate cancer screening recommendations. 2 Screening for prostate cancer: U.S. Preventive Services Task Force recommendation statement. Ann Intern Med. 2008; 149: 185-191 Crossref PubMed Scopus (486) Google Scholar Surveyed men were a highly selected group who attended an annual free prostate cancer screening clinic and agreed to complete a survey. Authors reported patient awareness—but not their knowledge of the USPSTF recommendations or the content of the screening clinic message—although presumably the clinic strongly favors screening given that free PSA tests are offered annually. Findings from their selected case series of men treated with radical prostatectomy do not provide reliable information on benefits and harms of screening, outcomes among men aged ≥75 years of age with newly diagnosed prostate cancer vs men aged <75 years, or comparative effectiveness of early intervention vs observation. Wilt and Calonge, both distinguished members of the United States Preventive Services Task Force (USPSTF). I know and respect Dr. Wilt, having served with him on various committees and panels over the prostate-specific antigen (PSA) era. However, I disagree with the USPSTF recommendation regarding PSA testing in older men. Full-Text PDF
