Abstract

Background. Surgical tactics in elderly patients with acute cholecystitis is still widely discussed. Materials and methods. The results of different management of acute cholecystitis in 352 elderly patients with high operational and anesthetic risk are represented. Patients formed 3 groups. A study group comprised 113 patients, in which treatment is-use minimally invasive methods under ultrasonic control. The first group consisted of 163 patients who underwent surgery, the second group of comparison of 76 non-operated patients. Five-year survival was studied. Results. During the first year died 24 operated patients, 18 non-operated patients and 8 patients of the main group. Within 5 years, died 50% of operated patients, 97.3% (p <0.05) non-operated patients and 39% (p <0.05) in the study group. There was no significant differences in the causes of deaths after the first observation. Conclusions. The study showed, that surgical tactics in elderly patients with acute cholecystitis using minimally invasive decompression techniques improves as proximate, as long-term outcomes.
